Appointments can now be made online please speak to one of our receptionists to register for this service.

Prescriptions

We are only able to accept repeat prescription requests in writing.

For your convenience the right hand side of the prescription contains a tick box list of all the medicines that are on your repeat list.

Tick the ones that you need and hand, post or fax the form into reception.

Your prescription will be ready for collection 48 hours later.

Currently both the chemists in Middlewich will collect repeat prescriptions from the surgery. If you wish to take advantage of this service, please write which chemist you would like to use on the form.

We can now take prescription requests from the web site for registered users (to register collect a form from the surgery- if you have signed up for online appointment booking then you are already registered)

The system will show you a list of the items your doctor has authorised for repeat issuing, simply select the ones you require.

Only items on the list can be ordered online.
